The photos show this patient has missing teeth immediately posterior to the lower lip lesion (1). The response to photodynamic therapy is impressive. It is very likely the patient has significant periodontal disease and poor oral hygiene, which are strongly associated with cigarette smoking. It would have been interesting if the authors had cultured the gums and periodontal tissues, behind the lip lesion. Organisms associated with periodontal disease are strongly associated with oral cancer as well as other malignancies (2,3,4). The combination of cigarette smoking, sun exposure and periodontal disease is a ‘perfect storm’ for developing this precancerous lip lesion.
