    Contexte : On déplore un important fossé sur le plan de la pratique, entre les soins optimums et les soins réellement prodigués aux patients victimes d’infarctus aigu du myocarde dans les hôpitaux du monde. Nous avons entrepris le projet de dresser une liste à jour des indicateurs factuels qui nous permettent de mesurer et d’améliorer la qualité des soins prodigués à cette population de patients.

    Méthodes : Un comité d’experts composés de 12 membres s’est réuni en 2007 pour dresser une liste à jour des différents indicateurs de la qualité des soins prodigués aux patients victimes d’infarctus du myocarde. Le comité a établi une liste de ces indicateurs potentiels après avoir passé en revue la littérature scientifique, les directives de pratique clinique et autres critères de qualité publiés. Pour dresser cette nouvelle liste, le comité a évalué chaque indicateur potentiel sous quatre aspects (fiabilité, validité, faisabilité et utilité pour l’amélioration du pronostic) et a débattu des indicateurs de qualité les plus importants à l’occasion d’une rencontre de consensus.

    Résultats : Un consensus a été atteint relativement à 38 indicateurs de qualité : 17 indicateurs seraient mesurables à partir de données extraites des dossiers médicaux et 21, à partir des données administratives. Parmi les 17 indicateurs liés aux dossiers médicaux, 13 concernent les traitements pharmacologiques et non pharmacologiques offerts aux patients à l’hôpital. La mortalité hospitalière a été recommandée à titre d’indicateur pronostique clé. Trois indicateurs liés au système de santé ont été recommandés pour mesurer la capacité de ce dernier à réagir promptement aux demandes d’interventions impliquant une collaboration. Les auteurs recommandent aux hôpitaux de tenter d’atteindre un objectif d’au moins 90 % des normes visées par les indicateurs du processus de soins.

    Interprétation : Les médecins et les hôpitaux pourraient sauver la vie de nombreuses personnes victimes d’infarctus du myocarde par la mise en �uvre de stratégies visant l’atteinte de points de repère ciblés en matière d’indicateurs de qualité.
