At the time, he was enlisted in the Royal Canadian Navy Volunteer Reserve and was posted overseas as a surgeon lieutenant commander on HMCS Prince Robert, where he remained for the duration of the war. While her husband was overseas, Mrs. MacMillan gave birth to their first child, Margaret MacMillan, now a historian and warden of St. Antony's College, Oxford. Four more children followed, Ann, a London-based CBC broadcaster; Tom, a financier; Robert, a urologist; and David, an energy consultant. After Dr. MacMillan was demobilized at the end of the Pacific War, he was joined by his growing family where he did postgraduate studies in London and Oxford, and qualified as a Member of The Royal College of Physicians in 1947. The next year, the family moved back across the Atlantic so he could take a position at the Toronto General Hospital as senior intern in hematology. He became a fellow of The Royal College of Physicians and Surgeons of Canada in 1948 and began his long career as a cardiologist on staff at the Toronto General Hospital. In 1962, Dr. MacMillan, along with Dr. Kenneth Brown established the world's first coronary care unit at the Toronto General Hospital. Within 1 year, they had reduced the mortality rate by 10%. Despite this medical breakthrough, his calling was not primarily as researcher. He established an extensive private practice and taught generations of medical students at the Toronto General Hospital and the University of Toronto.
