- Appendix 1: Essential tremor in a 77-year-old woman. This patient has rhythmic, involuntary oscillations of the upper limbs and head. Timeline 0 to 11 seconds of the video clip: minimal resting tremor over her hands, which is exaggerated when the patient outstretches her arms (postural tremor). The postural tremor is moderate in frequency and amplitude. There is no substantial latency (time lag) between the resting tremor and the appearance of postural tremor when the patient outstretches her arms. Timeline 12 to 26 seconds of the video clip: head tremor can be observed. Timeline 27 to 34 seconds of the video clip: this segment depicts vocal tremor.
- Appendix 2: Tremor in a patient with Parkinson disease showing latency between the resting tremor and the appearance of postural tremor. Resting tremor is present over the patient's right hand. When the patient outstretches his arms and maintains a position against gravity, the tremor disappears initially. After a latency (time lag) of about six seconds, postural tremor appears. This is in contrast to the video shown in part one of this series,1 where the patient with essential tremor had no substantial time lag between the resting tremor and the appearance of postural tremor. - Reference 1. - Appendix 3: Dystonic tremor in a 36-year-old woman. The patient has a moderate to high amplitude tremor in the neck. The tremor has some jerky character, predominantly in a "no-no" direction. Tilt of the head toward the left (left laterocollis) and posterior tilt (retrocollis) is seen.
- Appendix 4: Primary writing tremor. Tremor of the hand is observed while the patient is writing. No abnormal posturing of the hand is noticed (as seen in dystonic tremor of the hand). The patient did not have tremor in any other activity.
