Attendre plus longtemps ou changer de chirurgien?

La plupart des patients ne choisiraient pas de remplacer leur chirurgien par un autre dont la liste d'attente est plus courte, malgré les longs délais d'attente pour une chirurgie. C'est ce que révèle une enquête par questionnaire menée auprès de 1200 patients en attente d'une arthroplastie de la hanche ou du genou en Saskatchewan. Voir Recherche, page 327

Inhibiteurs de la pompe à protons

Une importante étude d'observation fondée sur des données de réclamations administratives révèle que l'utilisation des inhibiteurs de la pompe à protons pendant 7 ans ou plus est associée à un risque accru de fractures ostéoporotiques. Les auteurs ont également constaté un lien entre les fractures de la hanche et une exposition pendant au moins 5 ans à ces médicaments. Voir Recherche, page 319

Dans un commentaire relié, Richards et Goltzman déclarent qu'au moment de peser les risques et les avantages des inhibiteurs de la pompe à protons, les médecins et les patients devraient tenir compte des preuves qui s'accumulent et qui suggèrent que ces médicaments augmentent le risque de fractures. Anémie des maladies chroniques

Zarychanski et Houston avancent un argument solide selon lequel l'anémie des maladies chroniques ne serait pas une conséquence indésirable de la maladie systémique, mais plutôt une réaction bénéfique et adaptive à l'affection sous-jacente. Voir Analyse, page 333

Carcinomatose lymphangitique pulmonaire

Une femme de 31 ans présentait un essoufflement et une toux non productive, dont elle souffrait de façon intermittente depuis 1 mois. Le diagnostic initial a été une bronchopneumopathie chronique obstructive, mais il a été déterminé par la suite qu'elle était atteinte de carcinomatose lymphangitique pulmonaire et d'un cancer du côlon.
